How to File a Grievance
In order to view and print these forms, you must have "Adobe Acrobat Reader" installed on your computer. Acrobat Reader is a free program that can be downloaded by clicking the icon to the left and following the instructions.Once you have Adobe Reader installed, clicking any of the form names above will load the reader into you browser. The form will then be displayed so you can print it. When you are finished, just click the "Back" button on your browser to return to this page.

Employees working under the BellSouth Telecommunications, BASC, BellSouth Utilities, BellSouth Internet Services (dot Net) and BAPCO contracts have 60 days from the day they discover the contract violation to have the grievance met on at the informal level and file for the 2nd step (formal level). You can file a grievance by printing and filling out the following two forms and faxing them to the Local at 404.688.6236.

This Form documents the contract violation. This is where you provide the details of your case. Your signature on the form gives CWA the authority to view your records on your behalf. It ensures that we can contact you and your managers. It also specifies the grievance you are filing, the remedy you are seeking and the job steward you want to handle your grievance. The more information you provide means we can start processing your grievance sooner instead of researching who you report to.
